Sounds like a virus or a bad drive!
In case of the drive try LowLevel Format, all gets shaken bit by bit, takes hours to complete and your disk is like new again, I did that to drives who were ready for the trash and in most cases they got a new life.
it's inside some Mobo's Bios, just look, or at google: install it at a floppydisk and start the process
At Maxtor You might download: Powermax it's a disc utillity and it's Floppy sized and does all, brand of the disk is not important it supplies all brands!

Update:
Gangsta4christ: got no floppy disk drive
Riso:
Place a usb stick in to Your Pc and format the stick in windows, choose: format with: make a systemdisk and install Lowlevel Format or PowerMax
Works 100% but Your usb disk needs to be Bootable
adjust your Bios settings to let Usb Boot at start-up

I recommend using the free downloadable version from hitachi  (Drive Fitness Test, AKA DFT)

That one works on almost all drives I have thrown at it.
